Medical Plans
LOTT purchases its employee medical and dental plans through the Association of Washington Cities (AWC) Benefits Trust. This arrangement allows LOTT to join with other public employers to get the highest quality insurance coverage at the most reasonable price. LOTT pays 100% of the employee medical premium and pays 90% of the premium for qualified dependents. Our employees may select from two excellent medical plans:
Regence BlueShield AWC HealthFirst Medical Plan The HealthFirst Plan allows enrollees to access care from preferred providers for a $10 copayment. Care from other participating providers is paid by the plan at a reduced rate. Prescriptions are covered on a copay basis at participating pharmacies according to the following schedule: $4 generic copayment; $15 brand name copayment; $35 non-formulary brand name copayment. Please visit the RegenceRx website to determine if a specific medication is on the plan formulary.
Group Health Cooperative Plan The Group Health Cooperative Plan, which is a Health Maintenance Organization, provides most within-network care for a $5 copayment; there is no annual deductible for this plan. Review this brief summary of benefits and limitations.
Dental Plan
LOTT pays 100% of the dental premium for employees and their qualifying dependents. Dental coverage is through Washington Dental Services and is an incentive-based plan that also includes a $1000 orthodontia benefit. The incentive plan encourages enrollees to obtain regular dental care by providing a higher level of coverage for patients who visit their dentists annually. When first enrolled in the plan, the "incentive level" (or payment level) is 70% for routine care. Each calendar year that the enrollee uses the dental benefits, the incentive level increases by 10%. If the dental plan is unused for a year, the incentive level will decrease by 10% for that enrollee, down to a minimum of 70% coverage.
Vision
LOTT provides full family vision coverage to all benefits-eligible employees and their covered dependents. Vision insurance is purchased through the AWC Benefits Trust and is provided by Vision Service Plan at the $25 copayment level.
